What is a GCIC form?
A GCIC form is a form required to be submitted to obtain a George Statewide Criminal history search. A Georgia Statewide Criminal search cannot be performed without this form completed by the applicant being screened.
Why is it needed?
The GCIC Consent Form is used to consent to a GCIC background check. GCIC stands for Georgia Crime Information Center, and it is responsible for providing criminal history information for the state of Georgia. The form is needed to get results from all Georgia counties rather than just the minimum ones that appear in the candidate's previous history.
What is required to be filled out on the GCIC form?
The GCIC form must have:
- Full Name
- Social Security Number
- Date of birth
- Full Address including City, State and Zip Code
- The appropriate boxes for Sex and Race must be checked off
- The consent box must be checked off
- The form must be signed and dated (within 30 days)
The box for 'Employment' must be checked off under 'Purpose Code' (No other boxes may be used for this section
